I think the main reason is that De Kuip was the venue of choice for concerts at that time. and they played it in '93 as well. Arena was open for less than a year and was only starting hosting its first concerts. And I don't know when U2 started booking the venues..
But I'm just guessing here..

Well that's interesting. I know they had problems with Italian promoters during Zoo TV. Other artists also had problems like this with the Italian concert business. The Rolling Stones didn't even play Italy on Voodoo Lounge or Bridges to Babylon tours because they were sick of getting fucked over.
I also read once that U2's management didn't want U2 to play Sarajevo, and were trying to get them to play Basel, Switzerland on that same date.
